Duties:
Mercer Reunification Services (MRUS) is designed to provide family engagement and supervised visitation services to Mercer County consumers referred by the New Jersey Division of Child Protection and Permanency (DCPP). We seek to help families restore or develop social functioning by promoting positive parenting and by strengthening attachments between children and their caregivers. Our goal is to avoid future incidents of abuse/neglect of children and to decrease the amount of time children spend in foster care. Services include case management, therapy, peer facilitated parent support groups, parenting skills training, and supervised visitation.
The Care Coordinator provides outreach, supportive counseling, case management, transportation, and psycho educational services to consumers. Maintains contact and coordinates with family, court, DCP&P, and other treatment providers/significant others. Demonstrates the knowledge and skills necessary to provide care appropriate to any age-related needs of the consumers served. The individual demonstrates knowledge of the principles of growth and development appropriate to the consumer population served. He or she must be able to assess and interpret data about the consumer’s status in order to identify each consumer’s needs and provide the appropriate care, including age-related care, needed by the consumer group. Responds to client crisis with prescribed procedures for de-escalation, crisis management and behavior management. Demonstrates a trauma-informed response to clients based on Safe Crisis Management guidelines. Communicates observations both verbally and in the client’s electronic health record using prescribed formats and terminology. Participates in and accepts supervision and feedback as scheduled with Supervisor. Attends internal and external trainings in pursuit of professional development and career growth. Follows
Agency policies and procedures regarding work practices and expectations. Excellent navigation skills and proficiency in using navigation applications to find locations. Time management and organizational skills to keep track of visits and stay on schedule to meet program guidelines. Ensures the children have good hygiene and changes diapers and feeds infants/toddlers as necessary.
Education/Experience:
Bachelor’s degree in mental health field, social work, or related field. Case management experience required. Bilingual English/Spanish preferred.
