Legacy provides housing and treatment for mentally ill adults and case management for mentally ill adults who are homeless or at risk of being homeless. We offer short-term crisis residences to assist individuals in Burlington and Ocean Counties struggling with a psychiatric crisis avoid hospitalization. We also provide community case management services for individuals with mental illness struggling to maintain their housing.
Community Support Services
Community Support Services offers a variety of supports to help Burlington County adults with a serious mental illness remain living successfully in the community. Services include case management, housing assistance, supportive counseling, advocacy, service linkages, illness management, peer support, life skills, education/employment assistance, transportation and 24-hour on-call support.
Enhanced Community Support Services is also available for adults being released from state psychiatric hospitals. This program offers a higher level of support and case management.

Fastow Supported Housing (Aging Out Youth)
Our Fastow Supported Housing Program offers affordable apartment housing in conjunction with case management, independent living skills instruction, post-secondary educational advocacy, job readiness preparation, and other supportive services for youth aging out of the child welfare system.

Community Residences for Children
Our Community Residences for Children serve youth age 11-19 with emotional challenges. In addition to housing, these five-bed homes provide highly structured programming including therapy, psychiatric services, recreation, life skills, education assistance and socialization. An interdisciplinary service team utilizes a variety of evidence-based practices to facilitate recovery so that youth can become healthy, contributing members of society. Residences are located in Burlington County, Camden County, Essex County, Gloucester County, Mercer County, Monmouth County, Ocean County, Salem County and Somerset County.

Therapeutic Foster Care and Reunification Services
Sometimes children are unable to live at home or have been removed due to abuse and neglect issues. Legacy Treatment Services operates multiple programs that aim to achieve family reunification whenever possible through therapy, case management and parent education. Services include therapeutic foster care placements throughout New Jersey, residential youth programs in twelve counties and a Mercer County community-based reunification program.
Kairos House
Kairos House is a short-term residential program located in Burlington County that offers a safe, therapeutic option for those who are at risk of hospitalization due to exacerbation of their mental illness; or for those who need further stabilization and/or a transition period following an inpatient stay.
Services include case management, treatment, housing assistance, wellness groups and life skills training. Kairos House residents are typically referred from Legacy’s Screening and Crisis Intervention Program. For more information, please call 609-261-3034.
Crosswinds Peer Respite
Located in Ocean County, Crosswinds is the first adult mental health respite to open in New Jersey. The program offers a safe, supportive environment for adults to stay on a short-term basis when experiencing or on the verge of a mental health crisis. Crosswinds offers peer support, case management, education, support groups, medication services, Wellness Recovery Action Planning (WRAP) and follow-up support. It serves as a crisis alternative that aims to avoid the repetitive cycle of psychiatric hospitalization.

PATH (Projects for Assistance in Transition from Homelessness)
Projects for Assistance in Transition from Homelessness (PATH) offers community-based case management and outreach services to Burlington County adults who are dealing with a serious and persistent mental illness and are homeless or are at risk of becoming homeless. This program also provides targeted case management services for military veterans experiencing homelessness and mental health issues.
Individuals may be referred by a county agency or hospital, or may contact PATH directly for information about eligibility.
