Integrated Care Clinic
The Integrated Care Clinic is a Certified Community Behavioral Health Clinic (CCBHC). As a CCBHC, the Integrated Care Clinic provides evaluation services, outpatient therapy and medication management services, case management services, and peer support services. Services are provided for consumers looking for assistance with mental health, addictions, or both. Our focus is on providing integrated care—working with the consumer as a team for their mental health, addictions, and physical health care needs. The program serves both youth and adults. Services focus on providing person-and family-centered care from crisis prevention, to coordinating crisis response with local resources, and crisis post-vention services.
