Legacy’s Community Based Support Program, PATH, helps individuals and families who are homeless or at-risk of homelessness secure support. PATH (Projects for Assistance in Transition from Homelessness) offers community-based case management and outreach services to Burlington County adults who are dealing with a serious and persistent mental illness and are homeless or are at risk of becoming homeless. This program also provides targeted case management services for military veterans experiencing homelessness and mental health issues. Recently, Legacy received generous grant funding from WOMEN’S WAY, allowing us to provide more emergency housing stays to those served through this program.
One grateful consumer attests that the PATH program has helped her find much needed stability in her life. Chronically homeless for over 15 years, K. often ran out of money paying for motel stays in attempts to keep a roof over her head. Thanks to the PATH team and funders such as a WOMEN’S WAY, Legacy was able to connect K. with temporary housing, help her successfully manage her medication, and transport her to much needed doctor’s appointments. By alleviating the financial burden of temporary housing, K. was able to save some of her resources to help prepare her for the next steps on her journey to thriving. Legacy also assisted K. in her interview and application process for a new job, which she will be starting this month.
