Duties:
Contributes to the development, implementation, monitoring, and updating of consumers’ individual habilitation plans; therapeutic rehabilitative skill development; implementation of Illness Management and Recovery (IMR) principles and interventions; coordinating and managing services, both mental health and physical health; crisis intervention, as needed. Provision/coordination of services aimed at active rehabilitation and support services to achieve restoration of functioning and the achievement of community integration; maintaining own appointment schedule and meeting work productivity requirements; other duties as assigned by management staff.
Education/Experience:
Bachelor’s Degree from an accredited college or university in a mental health discipline, including social work, human services, counseling, psychiatric rehabilitation, psychology or criminal justice. For staff with a Bachelor’s Degree in a field other than those listed above, a minimum of 2 years working in a community based behavioral health setting. Certified Psychiatric Rehabilitation Practitioner (CPRP) may be substituted for one year’s experience. Experience working with the DD-MI dually diagnosed populations desired.
Hours
Full Time; Monday – Friday, 7:00 a.m. – 3:00 p.m.
